About the Role


As a Care Coordinator – Aged Care, you’ll be responsible for designing and managing individual care plans that align with each client’s personal goals, health needs, and lifestyle preferences.


Key Responsibilities Include:


  • Conducting face-to-face assessments and updating personalised care plans
  • Aligning services with the Aged Care Quality Standards and client rights
  • Ensuring participants receive appropriate funding and support services
  • Leading and supporting a team of Support Workers
  • Promoting independence, safety, and well-being

What You Bring to the Role


  • Solid understanding of Aged Care Quality Standards and the upcoming Aged Care Reforms (July 2025)
  • Demonstrated experience managing aged care caseloads
  • Strong communication and people skills
  • Exceptional organisational and time-management abilities
  • Confidence in assessment processes and care plan development
  • Familiarity with the Support at Home program

What You’ll Need


  • Degree in Nursing, Social Work, or related field (or equivalent experience)
  • Minimum 3 years’ experience in aged care or community care coordination
  • Current Driver’s Licence and insured vehicle
  • Valid First Aid & CPR Certificate
  • Cleared National Criminal History Check and WWCC (or willing to obtain)
