FOCUS Connect, an approved My Aged Care provider based in Campbelltown, delivers government-funded CHSP services to older Australians across South West Sydney and Northern Sydney suburbs. We provide practical assistance, social connection, and tailored support so you can remain safe, active, and independent at home.
About the Commonwealth Home Support Programme (CHSP)
The Commonwealth Home Support Programme (CHSP) provides entry-level services for older Australians who can live independently but need some help at home.
CHSP includes domestic assistance, respite, transport, personal care, and social support. Services can be short-term, ongoing, or intermittent, depending on your needs.
FOCUS Connect delivers CHSP services across South West Sydney and Northern Sydney Suburbs, including Wollondilly, Wingecarribee, Campbelltown, Bankstown, Camden, Fairfield, Liverpool, Ryde, Hornsby, Pymble and surrounding suburbs.
The Commonwealth Home Support Programme (CHSP) is for older Australians who meet the following criteria:
- Want to stay in their own home but need low-level support
- Can live independently but benefit from help with day-to-day activities
- Do not require complex or intensive aged care needs (these are usually covered by Support at Home, formally HCP)
CHSP is about helping you maintain independence, dignity, and quality of life, while staying active in your community.

Commonwealth Home Support Program (CHSP)
Am I eligible for CHSP?
CHSP is for people 65+ (or 50+ for Aboriginal and Torres Strait Islander people) who can live independently but need low-level support at home. From 1 Nov 2025, eligibility also includes people 50+ who are homeless or at risk, and existing clients approved before 1 Nov 2025.
Will my CHSP services change on 1 November 2025?
If you’re already registered with My Aged Care and assessed, you’ll transition automatically and the majority of clients will continue with their current services in line with their assessed need.
Do I need to do anything before 1 November 2025?
Yes—make sure you’re registered and assessed with My Aged Care by 31 October 2025. If you’re waiting for an assessment on 1 Nov, your approval may be backdated (this depends on your circumstances). Call My Aged Care on 1800 200 422 to organise an assessment.
How do I apply for CHSP?
- Call My Aged Care 1800 200 422 or visit the website to register.
- A Regional Assessment Service (RAS) assessor visits you at home.
- Once approved, nominate FOCUS Connect and we’ll help set up your services.
What if I haven’t been assessed yet?
You’ll need to be assessed and approved to continue receiving CHSP services after 1 Nov 2025. Please book an assessment now via My Aged Care.
How much will I pay?
CHSP is government-subsidised, and you’ll be asked to make a small client contribution per service. FOCUS Connect’s current contribution rates (as at October 2025) are:
- Personal Care — $15/hr
- Domestic Assistance — $15/hr
- In-Home Respite — $11/hr
- Social Support Groups — $15 per session
There is no income test, and your Age Pension is not affected. Rates are reviewed periodically; concessions/financial hardship support are available, ask our team.
Can I change my services later?
Yes. CHSP supports can be short-term, intermittent or ongoing and can be adjusted as your needs change. Speak with your FOCUS Connect Care Coordinator anytime.
What if my needs increase beyond CHSP?
If your needs become more complex, we’ll help you move to the Support at Home program (formerly HCP) and support you through the My Aged Care process so you receive the right level of care.
