As an approved My Aged Care provider, FOCUS Connect is a not-for-profit organisation headquartered in Campbelltown committed to supporting seniors across Sydney's South West Suburbs. Our CHSP individual social support services help seniors engage with their community and feel socially connected, meeting their needs for social interaction and companionship.
CHSP Social Support - Individual
Through the CHSP program, FOCUS Connect provides individual social support with a dedicated companion who can assist you either at home or while you engage with your social contacts. This service is designed to keep you active and connected in community life across South West Sydney, including Campbelltown, Fairfield, Liverpool, Camden, Bankstown, Wollondilly, Parramatta, Blacktown, Penrith, and nearby areas.
We support you with attending social outings, exercise, health services, and shopping, ensuring you remain connected to what matters most. FOCUS Connect also offers culturally specific and special interest groups to suit your needs and interests.
Our transportation services are available so you don't need to depend on friends or family for travel.
FOCUS Connect will pair you with a compassionate support worker who understands your needs, helping you stay engaged with your community and providing meaningful companionship.
What Does It Cost?
If you're eligible for CHSP services in your home, you'll need to make a personal contribution. The Australian Government heavily subsidises our Commonwealth Home Support Programme, ensuring your fees remain affordable. Typically, you will contribute around $11 per hour for individual social support. Plus, there are no exit fees or hidden charges under the Commonwealth Home Support Programme.
