If you need support to live independently at home, you may be eligible for a government-funded Support at Home package. FOCUS Connect is an approved, not-for-profit My Aged Care provider that delivers services to eligible people across South West Sydney and Northern Sydney.

About Support at Home

From 1 November 2025, the Australian Government’s Support at Home program replaces the current Home Care Package (HCP) program. It’s designed to be simpler and more transparent, giving you more flexibility and control over the help you receive at home.

When does this happen?

  • Support at Home starts 1 November 2025.
  • Current Home Care Package clients move automatically under transitional arrangements at an equivalent funding level; services continue.
  • The Commonwealth Home Support Programme (CHSP) continues unchanged.

Key Changes Under Support at Home

The Support at Home program is designed to make aged care at home easier to understand and access. Click each heading to learn more about the key changes coming from 1 November 2025.

  • Automatic transition

    Current HCP clients move automatically to Support at Home; your funding level continues and your services carry on.

  • Your costs

    Your contribution settings don’t automatically increase because of the transition. If needs change later, your plan may be reviewed.

  • New funding levels

    Updated funding tiers provide more support where needs are higher.

  • Assistive Technology & Home Modifications

    Receive up to $15,000 for equipment, aids, or home modifications that enhance your home's safety and support your independence. (Some items may require clinical assessment.)

  • Simplified service categories

    All services fall under Clinical Care, Independence Support, or Everyday Living Support, making it easier to see what’s available.

  • Short-term supports

    Extra, time-limited help is available if your health or mobility changes (e.g., restorative care and an end-of-life pathway).

Who Is Affected

Grandfathered Clients

  • Approved for a Home Care Package on or before 12 September 2024
  • Stay on the current system for now
  • Contributions based on previous Home Care Package model
  • Protected by the “no worse off” guarantee

Hybrid Clients

  • Approved for a Home Care Package between 13 September 2024 and 30 June 2025
  • Start on the current system
  • Move to Support at Home automatically on 1 November 2025
  • Contributions based on the new Support at Home model

New Clients (from 1 November 2025)

  • Approved on or after 1 November 2025
  • Start directly with Support at Home
  • Contributions based on the new Support at Home model

Support at Home Services

The Support at Home program categorises aged-care services into three categories: Clinical Care, Independence Support, and Everyday Living Support. These categories make it easier to identify the available supports and determine how they align with your individual needs. At FOCUS Connect, we provide services across all three, helping you stay safe, active, and connected at home and in the community.

Additional Funding Options Under Support at Home

Alongside your regular care budget, Support at Home includes extra funding for specific needs.

Assistive Technology and Home Modifications (AT-HM):

Up to $15,000 for aids, equipment, and home modifications that make living at home safer and easier.

Short-Term Restorative Care:

Extra, time-limited support if your health or mobility changes, helping you regain independence after illness or a hospital stay.

New to My Aged Care?

If you’re new to aged care or would like to choose FOCUS Connect as your provider, you can nominate us when you receive your My Aged Care assignment letter. Simply use the referral code provided in your letter or contact our team for assistance.

Switch your Home Care Provider

Fees & Contributions (Support at Home)

We’re always upfront about costs, so you can make informed choices.

Support at Home services are government-funded, helping you access the services and support you need to live well at home. Depending on your circumstances, a contribution fee may apply. The exact contribution amount, if payable, is determined through an income and assets assessment by Services Australia. Some in-home and community services may have different rates for evenings, weekends, and public holidays. We’ll always be transparent about any costs so that you can make informed decisions about your services and support.

New Aged Care Act & Your Rights

The Aged Care Act 2024 puts your rights at the centre of aged care and introduces a clear Statement of Rights, beginning 1 November 2025.


As someone receiving aged care, you have the right to:

  • Make your own choices about your care, even if there’s risk.
  • Receive safe, respectful, trauma-aware services.
  • Have your culture, language, and personal background honoured.
  • Get support to understand your care options and raise issues safely.

Frequently Asked Questions

If you still have questions about what Support at Home means for you, our Aged Care Team is here to help. Here are some of the most common questions.

  • Will I automatically move to Support at Home?

    Yes. If you currently receive a Home Care Package, you will automatically transition to the Support at Home program at the same funding level. You do not need to reapply or provide new paperwork.

  • Does this affect CHSP clients?

    No. The Commonwealth Home Support Programme (CHSP) continues unchanged for now.

  • Will my fees or contributions change?

    If you are already on a Home Care Package before September 2024, you are considered a grandfathered client. This means you will not pay more than you currently do.


    If you are a Hybrid or new client, your contribution will follow the new Support at Home model, which is designed to be fairer and more transparent. Your Care Coordinator can explain what this means for you.

  • Do I need a reassessment?

    No. Your services will continue automatically. However, if your care needs or financial circumstances change in the future, your plan and contributions may be reviewed to ensure they stay fair and appropriate.

  • Will my services change?

    No. You will continue to receive the services you have now. Under Support at Home, services are grouped into three categories — Clinical Care, Independence Support, and Everyday Living Support — but your day-to-day supports will not stop.

  • What if I am on the National Priority System (waiting list)?

    When your funding becomes available, you will join under the Support at Home system at the level you were approved for.

  • How do I choose FOCUS Connect as my provider?

    If you are on the My Aged Care waitlist, you can nominate FOCUS Connect as your preferred provider when you receive your assignment letter.
